Our health maintenance plans are designed to help eliminate problems before they happen and make caring for your pet convenient and affordable. Consistent, quality healthcare is essential to your pet’s long and healthy life.
AVMC is committed to providing excellence in care. All pets are different, but they all benefit from consistent, comprehensive care. Each of our health maintenance plans provides veterinarian recommended care emphasizing disease prevention and early diagnosis, which are vital to your pet’s long-term health.

Feline Health Maintenance Plan
All plans include:
- 10% Off Services and Medication (Does Not Include Food)
All health maintenance plans require a $69 enrollment fee with the first monthly installment. Payments 2 – 11 are paid via automated withdrawal.
Senior Health Maintenance Plan (From ages 7 & up)
Annual Membership Fee: $825 / Monthly Installments: $68.75

- 3 Doses of Revolution Plus (Heartworm and Flea / Tick Prevention

Adult Health Maintenance Plan
Annual Membership Fee: $642 / Monthly Installments: $53.50
- Adult Wellness Blood Screen
- Ear Exam and Microscopic Cytology
- 6 Doses of Revolution Plus
- Complete Urinalysis Including Urine Sediment Exam
- Cystocentesis – Ultrasound Guided*
- Assesses Urinary Bladder for Stones and Other Abnormalities
- 2 Nail Trims
- 2 Dewormings
- Blood Pressure
Canine Health Maintenance Plan
All plans include:
- 10% Off Services and Medication (Does Not Include Food)
- 25% Off Annual Dental Cleaning for Adult and Senior Plans
All plans also include a $69 enrollment fee, due at the time of sign-up for the plan.

Annual Membership Fee: $750 / Monthly Installments: $62.50

Adult Health Maintenance Plan
Annual Membership Fee: $810 / Monthly Installments: $67.50

All health maintenance plans include a $80 enrollment fee with the first monthly installment.
Payments 2 – 11 are paid via automated withdrawal.
