Dental Consent Form

Please fill out this form as completely and accurately as possible so we can get to know you and your pet(s) before your visit. If this is an emergency, or if your pet needs urgent care, please call us at 248-478-5400 for a faster response.

Your pet is scheduled for a dental cleaning, evaluation, and any necessary procedures to treat, maintain, or restore good oral health today.

 

Several factors limit our ability to detect every problem your pet may be suffering from with just an oral examination. Many periodontal problems can only be detected when your pet is anesthetized for a full oral exam and possibly tooth x-rays.
